FDA recently licensed a new smallpox vaccine called ACAM2000 which is made by Acambis Inc. This new vaccine is not available for routine use, but will be stored in the country's Strategic National Stockpile. It could be used to inoculate people at high risk of smallpox exposure during a bioterrorist attack. FDA has issued a public health advisory about deaths and serious injuries that have occurred when pain-relieving transdermal patches containing fentanyl, such as Duragesic, were misused. This has caused fatal overdoses of the drug. The Institute for Safe Medication Practices describes an incident where a patient's caregiver placed the fentanyl patch on the patient's buttock, which was the site of her pain. A recent report from the Institute for Safe Medication Practices warns about the dangers of misprescribing fentanyl transdermal patches, such as Duragesic. ISMP reminds practitioners that these patches are intended only for patients who are opioid-tolerant, and should not be used for acute pain. ISMP also pointed out other prescribing errors. Several brands of fentanyl transdermal patches are being recalled because they may have defects that could allow the fentanyl gel to leak out of the patch. If the gel touches patient's or caregiver's skin, it could cause respiratory depression and possibly death.
